Some Wikimedians in various countries have created country portals that link to the Wikimedia projects. There are many different ways they've setup these pages, some include links to all the projects in one language, some just all the Wikipedias in the popular languages of their countries, and many other things.
This page is meant to catalogue all of these portals and to give you ideas (and best practices) if you want to setup your own.
at - Austria

Wikimedia Österreich maintains wikipedia.at, a portal with a box that searches different language Wikipedias. The portal allows visitors to search in German, Hungarian, Czech, Croatian, Slovakian, Slovenian, and Romani Wikipedias (all local languages used in Austria), but only displays the interface in German.
There is also a feedback page here on Meta-Wiki: wikipedia.at.
be - Belgium

Wikimedia Nederland maintains the Belgian portal, wikipedia.be. The webpage features the official and frequently-spoken languages of Belgium (Dutch, French, German, Walloon, Luxembourgish, Limburgish, West-Vlaams, and Kölsch). Although it also contains a search box, the main feature of the site is a localized "Welcome" message translated into every language the page supports. This message links to each Wikipedia version too.
ch - Switzerland

Wikimedia CH has a fairly extensive set of portals, one for each Wikimedia project. All of these portals have a search box and translated description for every Swiss-related language that we have projects in (English, German, Swiss German, French, Italian, Romansch). Look at the Wikipedia portal for an example.
Another interesting note is that the portals also display the CentralNotices that we are currently running.

Wikimedia Deutschland maintains wikipedia.de. The German portal searches only in German.

dk - Denmark

Wikimedia Danmark hosts a portal that searches the Danish, Faroese, Kalaallisut, and Icelandic Wikipedias. The portal includes a description in each language that links to the different Wikipedias and a search box.
fi - Finland

Wikimedia Suomi opted for a portal exactly like the main Wikimedia one: wikimedia.fi. This page link to Wikimedia Finland's official site and also every Finnish site of the Wikimedia Foundation.

Wikimédia France uses wikipedia.fr to search the French Wikipedia and the Wikimedia Commons. An interesting addition, however, is that the user stays on wikipedia.fr to view the search results.
il - Israel

The Israeli portal, wikipedia.org.il, looks to be hosted by an individual, rater than Wikimedia Israel. The page is just a search box (with Wikipedia's logo on it) that searches the Hebrew Wikipedia. However, there is also a "popular value" link that gives you a random frequently searched page that you can visit.

Wikimedia Indonesia hosts id.wikipedia.org or [1], which is just like the Bahasa Indonesia portal. The Indonesia site allows user to search both Wikipedia and Commons (keeping them on the site to view the search results). However, it gives users the option to search the Indonesia Wikipedia, the Bahasa Indonesia Wikipedia.
it - Italy
editAn Italian portal is not possible due to the well known issues (English summary) on which the Wikimedia Foundation did not act. Wikimedia Italia maintains several other domains which typically just redirect to a WMF-maintained subdomain or portal
nl - Netherlands

pl - Poland

Wikimedia Polska hosts wikipedia.pl, which is just like the French portal. The Polish site allows user to search both Wikipedia and Commons (keeping them on the site to view the search results). However, it gives users the option to search the Polish Wikipedia, the Kashubian Wikipedia, and the Silesian Wikipedia.
Polish Wikisource hosts wikiźródła.pl.
rs - Serbia

Викимедија Србије maintains wikipedia.rs. The page only consists of a localized search box and a langue selector with quite a few languages. In addition to having localizations in the Western South Slavic languages (Serbian, Bosnian, Croatian, and Serbo-Croatian), the portal is also displayed in (and links to) many other languages including English.
se - Sweden

Wikimedia Sverige maintains wikipedia.se. The page consists of a search box and a langue selector with 13 languages and a link to wikipedia.org. At the top there is a link to sv.wikipedia.org/, and at the bottom there are links to information about and privacy policy for Wikimedia Sverige. All domains hosted by Wikimedia Sverige can be found at a separate list on the chapter wiki.
Tips for creating new portals
editWould you like to have a portal for your country? The best way to go about that is by contacting your local chapters. If you don't have a local chapter, you should probably discuss this with other Wikimedians in your country to figure out the best way to layout the portal. There are many options to choose from, and some work better for different countries. If you need help, feel free to leave a message on the talk page.
