On this page you may find information about the Grant information form, which has a different version for individuals, groups, and organizations receiving grants. The purpose of this form is to confirm or request important information that will need to be included in your Grant agreement. If your grant is approved, please wait to receive instructions for completing this form from your grants administrator. It is important that you wait to hear from your grants administrator before completing this form.

Help with this form

The grants administrator will fill out many of the fields on this form that will closely match your funded grant submission, such as the official name of the request, the amount funded, and the grant term. Information from you may be needed about you or your organization, or about some of your grant's details. There are different versions of this form for groups, individuals, and organizations because we need to issue grant agreement differently for each type of grantee. The grants administrator will choose the correct form for your grant. You may read more about the information requested by this form here.

These fields apply to all grantees

  • Official project name. This is how you would like your project to be called in the grant agreement. It will probably match the project name listed in your funded grant submission. This will appear in an appendix to your grant agreement.
  • Grantee's legal name, official name of group, or official name of organization. Please include your name if you are receiving this grant as an individual, or your group or organization's official name if you are receiving this grant on behalf of a group or organization. This name will appear at the top of your grant agreement.
  • Grantee address. Please include a complete address with a country and a zipcode. This address will appear at the top of your grant agreement. For groups and organizations, this should be your group or organization's official address if you have one. Groups may use the address of one of the individuals signing for the grant if the group does not have an official address. If it is different from your mailing address, you should notify the grants administrator.
  • Total grant amount. This is the entire amount you are allocated for this grant. This amount should include this grant only, and not include other grants. This amount should be the currency in which you are receiving the grant, which in most cases should match the budget approved in your grant submission. For some organizations, this amount may be different from the amount you actually expect to receive. This amount will appear in the first paragraph of your grant agreement.
  • Grant term. This grant term is defined by your grant's start date and your grant's completion date and consists of the entire amount of time you are planning for or doing your project. Your grant term should match the dates in your funded grant submission, and should give a realistic idea of how long your project took. If you need to adjust the dates listed in your funded grant submission after it is approved, you can do so with permission from WMF. Your grant term will be listed in an appendix to your grant agreement and in the first paragraph of your agreement.
  • List the earliest date you might spend or have spent grant funds. We need this to define the "effective date" of your agreement. It is very important that we define this date correctly so that you will be able to use the grant funds to cover your project expenses. In some cases, the effective date of your grant may be different from the grant term listed on your approved submission. It will usually be different from the date at the top of your grant agreement, which is the date your grant agreement was actually issued by WMF. This date will appear in the first paragraph of your grant agreement.

These fields only apply to groups or organizations

  • Amount to be sent. In some cases, for organizations, an amount will be sent for this grant that is different from the total grant amount. This might occur if the organization needs to return money to WMF for any reason, including to return unspent funds from previous grants. If this field is different from the total grant amount, the grants administrator will show this on the form. This amount will not appear anywhere on your grant agreement, but it is important information.
  • Legal name of the representative signing this agreement. Groups and organizations should provide the legal names of anyone signing the grant agreement. This name or these names will appear at the top of the grant agreement with your organization's name, and must match the name or names of the person or people signing the grant agreement.
  • Role or title of of the person signing the grant agreement. organizations will need to provide the role or title of the person selected by the organization to sign the grant agreement. For example, "President," "Executive Director," or "Member of the Board of Trustees." This person needs to have the authority to sign this agreement on behalf of your organization. This title will appear at the top of your grant agreement with your representative's name.

3. The start date and end date of your grant must include a day, month, and year. These should be the same dates listed on your approved grant submission.

4. You may not use grant funds to pay for any expenses before the effective date of your grant, so it is important that we set your effective date correctly.

5. You may request your grant in your local currency. We are able to send most currencies, but not all. If we are not able to send you the payment in your preferred currency, we will send you an equivalent amount in another currency using the exchange rate on the date of your grant's approval.
