Help with this form

The information on this form is required to send the grantee a payment for a grant. This information is needed so we may comply with laws, regulations, and best practices around giving grants from the US, including checking individuals associated with this grant against the Specially Designated Nationals list. There are two versions of this form: one version for both individuals and groups, and one version for organizations. The grants administrator will choose the correct form for your grant, but you may read more about the information requested in each form here. If you need to list more than three individuals on Page 2 of this form, you may use additional forms and complete Page 2 only. Once you send this information to the grants administrator, you need only update it for new individuals. Sensitive information about grantees, including legal names and identification documents is stored on a password protected fileserver. We are happy to respond to questions and concerns about why this information is needed and how it is stored.

These fields apply to all grantees

Page 1: General questions

• Individuals signing the grant agreement. Please list all people who will sign the agreement.
• Individuals with access to the account. Please list all people with access to the bank account where grant funds will be received and kept.
• Bank document. Please attach a written document showing the individuals who can access the account where grant funds will be received or kept. This may be a copy of your bank statement if it shows who can access your account, or it may be a written letter from your bank. This bank document is not required for individuals receiving Travel and Participation Grants.

Page 2: List of individuals involved with this grant

If you need to list information for more than three people, please use a second copy of this form. You only need to fill out this form once, or each time you need to list a new individual.

1. Relationship of each individual to your project. This should explain why you are including the individual on this form. For example, they may be signing the grant agreement or may have access to the bank account.
2. Legal name of each individual. This should match the name included on this person's identification document. Please also include other names this individual may be known by.
3. Home address of each individual. This should be the home address of this individual (not your organization's address), and should include their current country of residence.
4. Date of birth of each individual. The official date of birth of this person, including a month, day, and year.
5. Please attach a digital copy of a government-issued photo identification document showing each individual's legal name.

These fields only apply to organizations

• Nonprofit status description. Please describe your nonprofit organization's status under local law. For example, “a 501(c)3 tax-exempt nonprofit organization,” or “a registered charity listed in my country's directory.”
• Nonprofit status document. Please attach a copy of an official document showing your organization is a nonprofit organization: (1) For US organization’s this will be a letter from the IRS documenting your tax-exempt status and your organization’s EIN. (2) For some organizations outside the US, this may be a document showing you are listed in a government registry for nonprofit organizations, or (3) an official copy of your organization’s bylaws stating you are a nonprofit, along with a document that recognizes your organizations incorporation under local law.

We must receive this information before issuing your grant agreement. Thank you in advance for your patience with this process, which we realize may be different from what is required in your home country. Please provide information about the following individuals on the second page of this form. Any person signing a grant agreement with us for this grant. Any person with access to the bank account where grant funds will be received or kept. You must promptly notify us if the list of individuals with access to your bank account changes during the course of your grant.

Important! To be considered complete, this form must include,

1. The completed second page of this form, with complete information for (1) any individual who have access to the bank account where grant funds will be received or kept, and (2) any individual who will sign the grant agreement. This page includes space to provide information about up to three people, but you may use an additional form to provide information about more people. This information is required for all grantees.
2. A written document showing the individuals who can access the account where grant funds will be received or kept. This may be a copy of your bank statement if it shows who can access your account, or it may be a written letter from your bank. This bank account document is required for all grantees except those receiving Travel and Participation support.
3. A copy of a valid government-issued photo identification document for each person listed on this form. This may be a national ID card or a passport, for example. This information is required for all grantees, and will be stored on a password-protected fileserver.

If one of the individuals listed here has already been identified to another Wikimedia project, we will still need you to provide their information and documents. We apologize for the inconvenience, but we are not able to share this information with other projects or programs because of restrictions around privacy. We will store all electronic documents on a password-protected fileserver and will not keep physical copies of these documents. If you have received a grant from us in the past and we have your information on file you do not need to provide it again: please contact the grants administrator to confirm that there have been no changes to your information.
